Services Havering commissions

Adult social care commissioned services support people with eligible care and support needs to live as independently and safely as possible.

These services may be arranged following an assessment and are designed to promote recovery, maintain daily living skills, support people in their own homes or provide appropriate accommodation-based care where needed.

Extra care

Provided by Care-Away Limited T/A Care Support, this service offers personal care and practical support to adults aged 55 and over who have an eligible care package and live in one of Havering’s extra care housing schemes.

Support is tailored to each resident and may include help with daily routines, personal care, medication, meal preparation and maintaining wellbeing.

The service enables people to live in their own self-contained accommodation, with on-site support available when needed, helping residents maintain independence while having reassurance that care is close by.

To be considered for Extra Care, residents will need to undergo a Care Act assessment and be assessed as having eligible care and support needs.

Where Extra Care is identified as an appropriate option, Adult Social Care will consider the person’s needs, outcomes, housing circumstances and suitability for available schemes before any placement is agreed.

Reablement

Provided by Essex Cares Limited, this short-term service supports Havering residents following hospital discharge or a loss of independence.

Staff work with people on practical goals such as washing, dressing, preparing meals, moving safely around the home and rebuilding daily routines.

The service is usually provided for up to six weeks and aims to restore confidence, improve independence and reduce or delay the need for ongoing care and support.

Eligible residents will usually be accessed through the Home First pathway, either in hospital or in the community, before reablement support is arranged.

The assessment considers the person’s immediate care and support needs and whether short-term reablement will help them regain independence and reduce the need for ongoing support.

The service is not available through self-referral.

Shared Lives

Provided by Bettertogether, Shared Lives offers family-based accommodation and support for people with eligible adult social care needs.

Approved Shared Lives carers welcome individuals into their own homes and provide long-term placements, short breaks, respite or daytime support depending on assessed need.

The service helps people build relationships, take part in community life, develop daily living skills and live more independently in a supportive household setting.

Residents will need to undergo a Care Act assessment.

A social worker will assess the person’s care and support needs, outcomes, risks and circumstances, and will consider whether Shared Lives is an appropriate option.

If suitable, the person may then be referred for consideration and matching with an approved Shared Lives carer.

Statutory and independent support services help residents to understand their rights, have their views heard, access urgent support when needed and share their experiences of local health and social care.

These services provide important safeguards and ensure that people are supported to participate in decisions about their care and support.

Adult’s Emergency Duty Team Service

Provided by NELFT, this service offers emergency adult social care and mental health support outside normal office hours across several north-east London boroughs.

The team responds to urgent safeguarding, welfare and mental health situations that cannot safely wait until the next working day.

It provides telephone advice, carries out assessments and visits where required, coordinates immediate interventions and refers cases to daytime services for continued support.

Statutory Healthwatch Service

Provided by Havering Mind, Healthwatch acts as an independent voice for people who use health and social care services in Havering. The service gathers residents’ experiences, promotes involvement in service design, provides information and monitors the quality of local provision.

It shares concerns and recommendations with commissioners, providers, scrutiny bodies, Healthwatch England and regulators so services better reflect local needs.

Community and Statutory Advocacy Service

Provided by Mind in Havering, Barking and Dagenham, this service provides independent statutory and community advocacy for adults who need support to understand information, express their views or take part in health and social care decisions.

Qualified advocates support people through assessments, care planning, safeguarding, mental capacity and mental health processes, treatment decisions and NHS complaints.

The service also offers early community advocacy for residents below statutory thresholds, helping people navigate complex systems, understand their rights and achieve greater choice and control.

Prevention services help residents to stay well, connected and independent for longer.

They provide early support in the community, reduce isolation, support unpaid carers and help people manage long-term conditions, disabilities or changes in their health and wellbeing before needs escalate.

Living Well Community Wellness and Empowerment Service

Provided by Tapestry Care UK, this service supports residents aged 18 to 64 years with long-term conditions or complex needs to improve their health, confidence, independence and connection to local activities and services.

Support is personalised and may include wellbeing planning, signposting, social connection, confidence building and coordination with community partners.

The service aims to address needs earlier, reduce isolation, prevent escalation and delay reliance on statutory care.

Ageing Well Community Wellbeing and Empowerment Service

Provided by Age UK RBH, this service supports older residents to remain active, resilient, socially connected and independent in their own homes.

The offer includes exercise and health activities, falls prevention, nutritional advice, social groups, home-based connection for isolated residents, benefits guidance, digital skills and support following hospital discharge.

It works with specialist partners to promote wellbeing and reduce the risk of needs escalating.

Support Services for Unpaid Carers

Provided by Imago Community, this service supports unpaid carers with information, advice, assessments, practical help, training and wellbeing support.

It helps carers understand their rights, access benefits and community resources, develop caring skills, plan for emergencies and take breaks where appropriate.

Peer networks, forums and personalised support help reduce isolation and burnout while supporting carers to sustain their caring role safely.

Dementia Support Services

Provided by Alzheimer’s Society, this service offers non-clinical support for adults awaiting a dementia assessment or living with a dementia diagnosis, as well as their carers, families and friends. Support includes early advice, practical guidance, emotional support and tailored wellbeing activities throughout the dementia journey.

The service helps people understand available support, build confidence, reduce isolation and remain safe and well at home for as long as possible.

All Age Autism Service

Provided by Sycamore Trust, this service offers community-based information, advice, outreach, peer support and activities for autistic people of all ages and their families.

Support may include help around diagnosis, mental wellbeing, education, transitions, employment, benefits, social connection and independent living.

The service also works with health services, schools, businesses and community organisations to increase autism awareness and support a more inclusive borough.

Community Equipment Service

Provided by Medequip, this service supplies equipment to help adults and children with disabilities, frailty or health needs remain independent and safe at home, in the community or at school.

Equipment can range from simple daily living aids to more specialist items such as beds, lifts, hoists and moving and handling equipment recommended following assessment.

The service supports hospital discharge, reduces risks to carers and helps prevent avoidable escalation of care needs.

Equipment will be assessed and recommended by an appropriate professional, such as the hospital reablement provider, the Occupational Therapy team, or another relevant health, social care or education professional.

Following assessment, suitable equipment may be arranged to support the person’s independence, safety and care needs.

Public health commissioned services focus on improving health, preventing illness and reducing health inequalities.

They include services that support sexual health, substance use recovery, healthy weight, smoking cessation, HIV prevention and early identification of health risks.

Pan London Sexual Health E-Service

Provided by Preventx Limited, this service gives residents access to discreet online sexual health testing, information and onward support without needing to attend a clinic initially.

It is delivered collaboratively across London boroughs and helps improve access, patient experience and consistency of sexual health provision.

The service supports earlier identification of infections and helps residents access appropriate advice or follow-up care where needed.

Integrated Sexual Health Services

Provided by Barking, Havering and Redbridge University Trust, this service offers free, confidential and open-access sexual health services across Barking and Dagenham, Havering and Redbridge.

The service includes contraception, testing and treatment for sexually transmitted infections, HIV testing and diagnosis, and specialist clinical advice.

It supports residents regardless of age or where they are registered for primary care and provides leadership across the local sexual health system.

Adult Drugs and Alcohol Service

Provided by Change, Grow, Live, this integrated service supports adults in Havering affected by alcohol, opiates, crack cocaine, other drugs or misused medication.

Support includes outreach, early intervention, harm reduction, specialist community treatment, criminal justice pathways, recovery planning, reintegration and relapse prevention.

The service works with partner agencies to support safer, healthier and more stable lives.

HIV Prevention and Support Service

Provided by Positive East, this service offers HIV prevention, diagnosis and support for residents of Barking and Dagenham and Havering.

Prevention work helps people make informed choices, while outreach and testing support earlier diagnosis and access to treatment.

The service also supports people living with HIV to manage their health, maintain wellbeing, build resilience and participate in their communities.

Long Acting Reversible Contraception Service

Delivered through participating Havering GP practices, this service provides access to long-acting reversible contraception, including implants and intrauterine devices where clinically appropriate.

Trained clinicians provide information and counselling on contraceptive choices, supporting residents to make informed decisions.

The service improves local access to highly effective contraception and can include intrauterine contraception for emergency use where suitable.

This service can be obtained by visiting your Havering GP practice.

Residents should contact their GP practice directly to ask about long acting reversible contraception options, appointment availability and whether the practice provides this service or can advise on the most appropriate local route.

NHS Health Checks

Delivered through Havering GP practices, NHS Health Checks provide eligible adults aged 40 to 74 with a preventive health assessment.

The check reviews key risk factors and identifies early signs of conditions such as heart disease, stroke, kidney disease, type 2 diabetes and dementia.

Residents receive personalised advice and, where needed, onward testing, treatment or referral to help reduce future health risks.

Residents can access NHS Health Checks through their Havering GP practice.

Eligible residents may be invited by their GP practice, or they can contact their GP directly to ask about availability, eligibility and how to book an appointment.

Community Pharmacy Services

Delivered through participating community pharmacies in Havering, this service provides accessible stop-smoking support, with a focus on communities experiencing greater health inequalities.

Eligible residents receive an assessment, behavioural support, practical quitting advice and appropriate stop-smoking medication during a structured programme.

The local pharmacy setting makes support easier to access and helps residents plan, begin and sustain a quit attempt.

Provided by DDM Health Ltd, this service offers community-based support to help residents achieve and maintain a healthy weight.

The service combines nutrition guidance, physical activity, mental wellbeing, personalised goals and evidence-based behaviour-change support, with both face-to-face and digital options.

It connects participants with local groups and related services so improvements can be maintained and health inequalities reduced.
